Our office has access to records for individuals who were born or who died in Douglas County, Nebraska. Birth records for individuals who were adopted at any time are not available through our office.

Office Hours: 8:00am - 4:00pm Monday through Friday
The office is closed for major holidays.

Note: Applications for Amendment of certificates must be started by 3:30PM.

  • Birth Certificates $17.00 each
  • Death Certificates $16.00 each
  • Amendments to Certificates $16.00 each

Who can request a Birth Certificate or Death Certificate?

  • Birth Certificate - Certified copies of birth certificates may be requested by: the person themselves, their parents, their children or their spouse. Please call if your situation is different.
  • Death Certificate - Certified copies of death certificates may be requested by: the informant on the record, the parents, children or the spouse of the deceased. Please call if your situation is different.

To Order by Mail:

  1. Download or open the relevant application from the links below.
  2. Fill out the application completely.
  3. Print the application and sign it.
  4. When applying by mail, please include a photocopy of the requester's current government-issued photo identification, a self-addressed stamped return envelope, and payment by check or money order payable DCHD.
  5. Mail to:

Associated Fees:

Include $17.00 for each certified copy of Birth Certificate ordered.

Include $16.00 for each certified copy of Death Certificate ordered.
